We are constantly striving to make your visits to our office as efficient and smooth as possible. To better serve you, our practice is transitioning to Phreesia, a secure, digital patient intake platform used by leading healthcare organizations nationwide.
Phreesia allows you to complete your registration forms, verify insurance, and handle balances from your own smartphone, tablet, or computer before you arrive for your appointment. This eliminates the need for clipboards and manual paperwork in the waiting room, getting you in to see your provider much faster.
You will receive a text message or email from our office containing a secure, personalized link to start your check-in.
Click the link and confirm your identity using a secure, multi-step verification process. Note: Phreesia does not require you to create an account, remember a password, or log into a patient portal.
Review and update your demographic information, fill out clinical questionnaires specific to your urologic health, and use your device's camera to snap a photo of your photo ID and insurance cards.
Review any applicable co-pays or outstanding balances and pay securely through the platform using your preferred payment method.
- Complete the forms early: Please submit your digital registration at least 24 to 48 hours prior to your appointment. This allows our billing team to verify your insurance coverage in advance and resolve any eligibility issues.
- Have your physical cards ready: Even though you will upload images of your photo ID and insurance cards via Phreesia, please bring the physical cards to the office for your first visit under the new system so we can double-check the data.
- Keep your contact info current: Ensure our office has your correct mobile phone number and email address on file, as Phreesia relies on these to send your check-in links.
- Do not empty your bladder immediately before arrival: Many urology visits require a routine urine sample. Unless instructed otherwise, plan accordingly when arriving at the clinic.
Phreesia adheres strictly to the Health Insurance Portability and Accountability Act (HIPAA). All of your personal health information is encrypted and securely transmitted directly to our electronic medical records system. Your data is never shared or sold.
