Tips for a Smooth First Visit

To provide you with the best care and minimize your wait time, please keep the following tips in mind as you prepare for your appointment:

  • Please arrive 15 minutes early if you have already completed your paperwork, or 30 minutes early if you still need to fill out your forms in the office. This allows us to process your insurance and get you into the exam room right on schedule.

  • We want to see you on time! Please verify which of our office locations your appointment is scheduled for. You can find addresses and Google Maps links

  • Your health—and the health of all our patients—is important to us. If you need to reschedule or cancel, please notify us at least 24 hours in advance.

    Why it matters: Giving us 24 hours' notice allows us to offer that time slot to another patient who may be waiting for urgent care.

  • In most cases, we will need a urine specimen at the start of your appointment. Please arrive with a relatively full bladder or ensure you have stayed hydrated throughout the day so you are prepared to provide a sample upon check-in.

    • Photo ID (Driver’s license or state ID)

    • Insurance Card(s)

    • Current Medication List (including dosages)

    • Completed New Patient Forms

    • Referral (if required by your insurance)
